Why is my toilet running non-stop, and how do I fix it?

Usually, the reason why toilets run continuously is that the flush toilet chain is loose and prevents the flapper (rubber seal) from sealing, but if the chain is too tight, it will also continually run as there might be a gap between the flapper and the bottom flush valve.
